Google Forms is the obvious free choice — unlimited forms, unlimited responses, automatic Google Sheets integration. It's not pretty, but it works. Tally is the better-looking free option: unlimited forms, unlimited submissions, and a document-style editor that makes creating forms feel like typing a doc.
Mid-tier: Typeform and SurveyMonkey
Typeform ($25/month) creates beautiful, conversational forms that feel more like a chat than a questionnaire. Higher completion rates, but expensive per response.
